Form 1: Creating a Course Page
Getting started: fill out one copy for each course for which you'll be posting items in ERes.

Form 2: Adding an Item to a Course Page
To add an item to a newly-created or existing course page, fill out one copy for each item that you are adding to your course page.

Form 3: Copyright Release
To verify copyright compliance, fill out one copy for each copyrighted item that you are adding to your course page. (If you are posting items that you have created yourself, or items that are in the public domain, this form is not necessary.)
