Required Forms

Once you have completed the steps to become a Dragon student-athlete, submit each of the following eligibility forms to the main office of the Athletic Department.  All forms must be in the office before or on the day of your first practice.  No/missing completed forms, no practice.

Transfer Students
Transfer student-athletes must have official transcripts from previous institutions to send to the college

*You can obtain a physical at the Athletic and Fitness Center if needed.  HCC hosts an evening each year for prospective student-athletes at a cost of $20.  See the Medical Forms Packet for more details.
