Submit the form at one of the following locations:

• Office of Records, Registration & Veterans’ Affairs (RCF-233; drop box or in person)

• Office of Admissions and Advising (RCF-242; drop box or in person)

• Main Welcome Center location (RCF-1st floor adjacent to the Bookstore; in-person only). 

Please bring your student ID to verify an address change.
