Permission to Disclose (Signing this form authorizes Financial Aid Services to discuss the student's file with a designated third party.) This form must be submitted in person by the student with a photo ID to Financial Aid Services, Room RCF 222. |
Default Form (This form is required by all students that are in default of one or more federal student loans. In order to borrow a new loan at HCC, the student must complete this form.) |
Dependency Override Appeal (This form is used to request a change in student dependency status. Appropriate documentation is required. Students must meet with a Financial Aid Counselor during designated counseling hours.) |
Request for a Professional Judgment (This form is used to request that extenuating circumstances not addressed on the students original FAFSA be considered when determining financial aid eligibility for the 2012-2013 academic year. Appropriate documentation is required.) |
SATISFACTORY ACADEMIC PROGRESS (SAP) |
SAP Restriction Appeal Form (This form is used to request consideration for reinstatement of financial aid awards by students who have been determined to be on financial aid restriction. Refer to the SAP webpage for more information.) Restriction Appeals for Summer/Fall 2013 are due by Friday June 14, 2013. Late appeals cannot be considered. |
Financial Aid Academic Plan (This link provides information on how to obtain a financial aid academic plan. An academic plan is required only for students who submit a restriction appeal request and receive a restriction appeal approval.) |
