Forms Library 2008-2009

The Forms Library includes forms for the 2008-2009 academic year. The 2008-2009 academic year includes: 2008 - Summer 2, Summer 3; 2009- Fall, Winter, Spring, Summer 1, Summer 1 Extended.

HCC uses document imaging to retain student information which makes handwritten documents difficult to read.
In order to serve you better, we request that you:

complete all forms by typing directly on the fill-in form using Adobe Acrobat Reader 8.0* or higher;
print, sign, and submit all forms along with all required documents by mail, fax, or in person to Financial Aid Services, Room RCF 222

VERIFICATION FORMS
The forms listed below are used to complete the review process called verification as determined by the U.S. Department of Education.
Please refer to your "Notification of Missing Information" to verify the forms that you are required to complete. You are not required to complete each form.
You must go to www.fafsa.ed.gov if you were asked to provide signature(s) or make adjustments to your SAR.

MISCELLANEOUS FORMS

Permission to Disclose
(Signing this form authorizes Finanical Aid Services to discuss the student's file with a designated party.)
Please go to Financial Aid Services, Room RCF 222 to obtain this form.
Special instructions apply.

Dependency Override Appeal
(This form is used to request a change in student dependency status.
Appropriate documentation is required. Students are also asked to meet with a Financial Aid Counselor during designated counseling hours.)

Request for a Professional Judgment
(This form is used to request that extenuating circumstances not addressed on the students original FAFSA be considered when determining financial aid eligibility for the 2008-2009 academic year. Appropriate documentation is required.)

Financial Aid Restriction Appeal
(This form is used to request consideration for reinstatement of financial aid awards by students who have been determined to be on financial aid restriction.)
The Restriction Appeals Committee is no longer accepting appeal request for Fall 2008.
The committee will begin accepting appeal request for Spring 2009 beginning December 1, 2008.

(This form is used to cancel or reduce existing student borrower loan amount(s) for the Fall08/Spring09.)

Work-Study Hiring Form
(Please refer to the "Apply for Work-Study" web link for details on how to apply.)

(This publication provides more information about HCC's Work-Study program and includes detailed policies and procedures for employers and employees.)

Request for Foundation and Development Work-Study (FDWS)
(This form is used to request on-campus employment by students who attend HCC on an F-1 Visa.)
All FDWS positions for 08-09 are currently filled. No additional request are being accepted at this time.

*You may experience technical problems (i.e., form appears blank, form will not print or prints blank) if you attempt use Adobe Acrobat Reader 7.0 or below. Users may receive an "Adobe Updater" requesting to download Japanese Fonts. Please click "OK" to download the update. The "Adobe Update Manager" will instruct the user to close the open Adobe application and than click continue to install. The user must reopen the document after the updates are complete.
