Get Adobe ReaderTo view the Portable Document Format files (PDF's) from this site, download a free Acrobat Reader. You will need to install the Acrobat Reader on your computer before viewing PDF files.  Some applications are available in a PDF format that allows you to save teh application and complete the document on your computer by typing in all the fields.  A hand-written signature is still required.  Applications and supporting documents may be submitted by mail or in person to the Office of Admissions & Advising (RCF-242).

CVT
This clinical program begins in Spring 2010.  The application period is July 1 - December 1, 2009.

Nursing
The application period for the Accelerated RN, Traditional RN, and LPN programs is September 15, 2009 - February 15, 2010.  Please note that applicants to these programs must take and pass a nursing admission assessment to qualify for the program.

The LPN Pathways application period runs year-round; specific deadlines exist for each entry semester.  Please see the application below for more information.

Radiologic Technology
The 2010 application period for Rad Tech is September 15, 2009 - February 15, 2010.  Please note that beginning with the Summer 2010 clinical program, PHYS-101 will be a co-requisite of the program.  Also, this program will now begin in Summer Extended.

MMAHEC
Applications for Mid-Maryland Allied Healthcare Education Consortium (MMAHEC) programs are available on the Carroll Community College and Frederick Community College websites during application periods.  Applicants to these programs must meet with an allied health advisor in the Office of Admissions and Advising to submit their application materials.

Nursing Testing Forms

Applicants to the clinical portion of HCC's nursing programs must take a nursing admission assessment as part of the application process.  Registration forms must be submitted with payment to the Office of Admissions and Advising.
