
If you are a student who has been readmitted into one of the Health Sciences Division programs, you are responsible to contact the Health Sciences Division Clinical Coordinators, so that they can discuss with you what you will have to do to update your health records. Linda Simpson St. Clair MA, RN, BSN Margaret Tricoli RN, BSN The HSD Clinical Coordinators need to know what semester you were last within a program, for example, Spring 2012 or Fall 2012. From this point, we can tell what needs to be updated. Items which may be requested are: current and correct CPR --American Heart Association for the Health Care Provider, BLS ( Basic Life Support); current TB screening ( either one step PPD or TB questionnaire); proof of the correct annual flu vaccine. Depending on the length of time you have been out of a program, you may need to do a ReCheck Criminal Background at a reduced rate of $ 26.00 and a new Urine Drug Screen at $46.00 with an updated physical exam. This process is to be done ONLY through PreCheck. You are responsible to contact the coordinators to find out what your specifics may be. |
